Community Forms

These forms are provided for the following purposes:

1. Community Engagement Checklist
   This checklist must be completed by the PTJ upon receiving an application from the community. It serves as a justification to determine whether the application is appropriate for implementation, with reference to the Community Engagement Implementation Guidelines (SOK.JINM.GP02/JARINGAN KOMUNITI).

   Please attach the community’s application letter together with this checklist.

2. Community Verification Form
   This form must be verified by the community representative only and does not require verification by the Head of the PTJ.
